PARATRANSIT APPLICATION FORM
 

To be eligible for the RTL’s paratransit service, you must :
  1. Fill out the application form (also available on request at 450-670-2992, option 3);
     
  2. Send your completed form to the RTL eligibility committee at the following address :

Only persons duly authorized by the eligibility committee can use the paratransit service. The eligible person must meet the criteria listed in the
paratransit eligibility policy of the ministère des Transports, de la Mobilité durable et de l'Électrification des transports.

Residents of specialized and/or subsidized institutions already eligible for a transportation service can use the RTL’s paratransit service for reasons other than medical ones.
 
 

 

 

UPDATING YOUR USER FILE

 
  Please let the RTL know if there are any changes to your user file :
  • Adress
  • Telephone number
  • Medical condition
  • Mobility aid (wheelchair, cane, walker, three- or four-wheel mobility scooter, guide or service dog, wheeled shopping bag, etc.)
  • Need of a personal attendant
  • Other
   Note that the Reservations Center cannot update your file
 
Your file will be automatically deactivated if you do not use paratransit for more than two years. Please contact us to reactivate your file.
